Some together: Left heart muscle ischemic, dead or scarred and does not push blood well( low ejection fraction), low blood pressure, and may back up and strain right heart and get pulmonary fluid retention, edema, peripheral edema. If right side muscle dies, scars or is under back pressure from bad lungs, pulmonary embolism it may show first signs, but left heart is not happy either due to poor inflow. Simple?
