In the setting of?: L:P ratio > 30 suggests inherited disorders of the respiratory chain complex or tricarboxylic acid cycle disorders. An elevated lactate and a L:P ratio less < 25 suggests a defect in pyruvate metabolism. Are you being worked up for congenital lactic acidosis?
