Uti: Simple urinary bladder infections usually don't include those complaints and I wonder if you're experiencing side effects from the antibiotic. Azithromycin is best taken with food to avoid this. It is not usually prescribed to treat UTI's as the common bacteria causing them are not sensitive to azithromycin in my experience although I don't know your urine culture and sensitivity results.
