Pakistan: Polio spreads the most in Pakistan. It is one of the three remaining countries in which polio has not been eradicated (the other two are Afghanistan and Nigeria). It spreads because it is contagious from person to person, and it is a challenge to get the entire populations in these countries adequately vaccinated.

Polio: There were only 22 reported cases of polio Worldwide in 2017. Per the World Health Organization only 3 countries still have cases of polio transmitted: Pakistan, Afghanistan & Nigeria. This can occur because not all children receive polio vaccines.
