Probably infection: a painful lump that fills up with fluid in that area is likely a cyst formed by infection of an oil gland that has become infected, and is now the scene of a struggle between your immune system, and the infection. Hot compresses with herbs like comfrey are the old fashioned way. Consulting your doctor and possibly antibiotics would be a very good idea.
