Does the chest x-ray impression directly say whether there is mass in the lungs or does it talk about only acute diseases?

Usually: If mass is present on chest exam, radiologist is trained to mention this as an abnormality. He is trained to differentiate abnormal from normal findings as well as as sometimes acute from chronic changes when comparing exams. Radiologist should mention all abnormalities that are present in report of examination.
