Essentially zero: A condom if used properly is a very effective contraceptive as long as it doesn't break. If it was stored and handled properly (not in a wallet, not in direct sunlight...) it is safe to assume that it is not defective. Plan B is also a very effective emergency contraceptive even after an encounter with no other protection at all, so with no ejaculation + use of a condom, that's too much actually.
