No one knew?: Perhaps incident in Residential Care Facility for Elderly was not witnessed, and resident didn't report trauma/fall/injury to caregivers? Painful chest & traumatized left shoulder might not be as easy to observe in passing. One would hope that someone would have observed difficulty walking & inquired. It also depends upon frequency of interactions w/staff. And of course, staff experience & trainin
