Anal muscles: Severe anal pain that is stabbing and incapacitating is often something called proctodynia or proctalgia fugal (pain from spasm of the muscles around the anus). The precise cause is still somewhat mysterious but there are medicines to help if the problem is very intrusive or persistent. Severe constipation can also contribute to anal pain/lower abdominal pain (e.g. anal fissures).
