Usually we will write "D&C" which is the procedure, which can be done for any number of things, an abortion just happens to be one of the reasons.

It depends on where: If the termination of a pregnancy is done for medical reasons at a large hospital, this will become part of your record. An elective abortion would not typically be shared if it were done at a small abortion clinic. One should check at the facility prior to the procedure to be sure.
