It should: A medical procedure performed at a medical treatment facility should be documented in the medical record.

Yes: A surgical procedure is a surgical procedure and should always be placed on your medical record in order to ensure that your future care providers have a full understanding of your history and how to treat other conditions as related to your procedure. Usually we will write "D&C" which is the procedure, which can be done for any number of things, an abortion just happens to be one of the reasons.

It depends on where: If the termination of a pregnancy is done for medical reasons at a large hospital, this will become part of your record. An elective abortion would not typically be shared if it were done at a small abortion clinic. One should check at the facility prior to the procedure to be sure.
