No ER: Spider bites can cause systemic symptoms. Generally this is not an emergency but he should get seen by a doctor.

Infection? Shingles?: People often assume spider bites for large, painful, or itchy red bumps, but spider bites are rarely the cause. It might be only a large pimple, but the other symptoms could mean more serious infection, or maybe herpes zoster (shingles), especially if more bumps appear or if there is pain outside the bump itself. He should get care soon. ER OK if severe pain or fever, otherwise call doc first.
