MTX lung disease: Lung disease from MTX occurs in about 1% of patients - more with pre-existing lung problems. There is an acute hypersensitivity pneumonia, which may resolve, and a slow, insidious lung fibrosis, considered irreversible. The drug must be stopped immediately and not re-tried. If one gets to it very early, some have used D-penicillamine. Dr Art Lorber at Long Beach Mem Hosp had some expertise here.

Methotrexate induced: Pneumonitis could be various degree from mild to severe. However, the most definitive way to diagnosed is from lung biopsy follow by blood work, chest X-ray and CT chest. The progression often slow down if not stop after discontinue methotrexate and more importantly how one respond to steroid.
