Condom failures: Condoms are manufactured under strict guidelines which vary with the type of material used. (latex versus sheepskin). That being said ,condoms can and do fail by leaking or rupture. A leak will not usually be detected by the user, and the "pull out" method does not account for the sperm that are indeed present in the "pre-cum". So, there is always a chance for pregnancy in your scenario.
