Cold sores are hollo: Typically on the outside at the border of the red. If it recurs in the same place then cold sore is just about the only possibility, otherwise dermatologic conditions are hard to diagnose without seeing them. Cold sores are hollow, fluid filled, and temporary--going into healing in under a week. Warts, moles, tumors, other infections don't heal up as fast and need treatment.
