Second opinion: The fourth metacarpal is not a very common area for a stress fracture. I would recommend seeing an orthopedic surgeon. A general orthopedic surgeon, a hand surgeon, or an orthopedic trauma surgeon should be able to evaluate you effectively. When dealing with stress fractures one needs to be evaluated for overuse and for metabolic issues such as vitamin D deficiency etc.

Metacarpal?: Perhaps you mean the metatarsal (in the foot)? A metacarpal (hand) stress fracture would be almost if not entirely unheard-of depending on your job. If you feel the referral is too far off, try touching base with the primary care doc who made the referral (sometimes they never are told how far out it is made for) who should be able to expedite it if needed.
