Vagal synope: Cough syncope or syncope during urination are exaggerated reflexes of the vagal nerve and can be stressful because trauma can occur. Training , avoiding certain situations and medication can be helpful to prevent or blunt the reflex Seizures are sometimes a separate problem and need further evaluation

Neurologist: Fainting during urination, defecation with straining is not uncommon. You can actually live without a vagus nerve - in the past, surgeons would treat peptic ulcers this way- but please see your neurologist with a written list of your questions.

3 areas: 1. Micturition syncope can be handled via many approaches. A urologist or neurologist could manage 2. The other provocations may be associated, but might see cardiologist to cover other possibilities. 3. Sounds likely that he has complex partial seizures, and hopefully, he is taking anti-epileptic meds. Wonder if a Concierge virtual appt can sort this all out.
