How do doctors know that women stop ovulating during pregnancy?

Good question: but tough to give a short answer in plain English. Basically high levels of any of 3 hormones: estradiol. progesterone, and prolactin will STOP ovulations in non-pregnant women (by disrupting pulses of GnRH and preventing LH surge.) In pregnant women these 3 are all very high.

Pituitary hormones: It takes cyclic secretion of pituitary hormones for ovulation to occur. During pregnancy and with the use of oral contraceptive pill the rhythm of the pituitary gland is disrupted and ovulation does not take place.
