If paired CMV IgG &: IgM titers document a primary CMV infection in the 1st trimester, a Fetal Maternal Medicine Specialist needs to follow your pregnancy. No treatments for CMV are safe for pregnant women. Chance of transmission to fetus is 30-50%; chance of neonatal symptoms, 10%; chance of symptoms by age 2, 20%. Baby will need regular Early Intervention assessments & tests of hearing & vision, if infected.

Complex situation: New CMV is uncommon at age 36 and the lab tests can be difficult to interpret, so the diagnosis of recent CMV could be wrong. Chronic or distant past CMV carries little danger for the baby. If recent CMV confirmed, possible treatment with antiviral drug or hyperimmune globulin injections. This needs sophisticated care, e.g. infectious disease specialist or ObG with expertise in CMV. Good luck!

Mgt: A maternal IgG avidity test for CMV can be helpful I the diagnosis of acute CMV. If there is confirmation of CMV in the amniotic fluid, treatment is usually initiated with Ivig.
