Patrick,: Trauma/injury would be the most common cause of testicular pain--typically you would know if that occurred. Epididymitis, inflammation of the tube that holds and carries semen and located at the back of the testicle, would seem most likely. Hernias, hydrocele, spermatocele or urinary infection also possible. Epidydymitis can be caused by (sexually transmitted?) infection. Needs testing/see Doc.

Agree with Dr Abbott: Dr Abbott provides a great starting differential. Beyond this, an examination is critical to begin to offer potential causes. If there is any discoloration of the skin, fever, or if the pain is increasing in frequency or intensity; then go to the ED.

Possible infection: Testicular pain without swelling and no urinary symptoms usually is due to infection. It is a good idea to have it checked out by a physician to determine the cause and treat it, possibly by an antibiotic.
