Toothbrush for sure: If someone is diagnosed with oral syphilis a new toothbrush is a good idea. Normal dishwashing will clean the utensils adequately.

Low risk, but...: I've never heard of this happening, and probably drying between uses of the toothbrush and toothpaste itself (or preservatives present in most brands) both would kill the syphilis spirochete. OTOH, it costs $2.00 or less to get a new toothbrush, and most people probably don't change toothbrushes as often as they should anyway. So why not?
