Need 2: About 20% of the people with anaphylaxis needs a second dose of EpiPen (epinephrine) either due to insufficient response or a 2nd phase of anaphylaxis occurring up to hours later. It is better to be safe than sorry.

2 is better: Anaphylaxis treatment can be unpredictable and sometimes requires 2 doses or more of epi. Hence, if the 1st dose isn't successful at reversing anaphylaxis symptoms, having another dose is great. At the same time, seeking immediate medical attention is important as sometimes, IV fluids are required if the EpiPen (epinephrine) is unsuccessful at elevating the blood pressure if the blood pressure is low.
