Hard to know: Gastric reflux or post nasal drip can both worsen inflammation in the throat at the position where the middle ear drains (and both become worse at night). This can worsen pressure in one or both ears. Could also be as simple as compacted ear wax. Physical exam is ideal, but simple virtual consult would likely be revealing.
