Lost filling: Be back to the dentist ASAP and have the filling replaced. There are a number of reasons that causes poor bonding and retention.. There may have been some contamination with moisture while the filling was being placed, you bite was traumatic, the sized position of the filling, clenching and grinding, habits, unhealthy gums tissues, are some of thecausesof failure

Call your dentist: If you can dislodge a filling by flossing then it needed to be replaced. See your dentist for exam.

Depends: Depends on: material used, where it was placed, prep design, time between placement/eating (didn't set up fully) etc. It happens to the best of us. Just call the office & let them know.
