ER: With systolic pressure of 210 I recommend that you go to the ER to get checked out. There is risk for stroke at this pressure.

Elderly hypertension: Typical goal blood pressure (BP) is less than 140/90 for most (younger) adults while we try to keep BP less than 150/90 in elderly. In the short-term, we worry more about excessively low BP rather than long-term complications in elderly. Even if no headache or chest pain, 90yo w/extremely high BP needs quick urgent evaluation as can be provided in emergency department. At least see FamilyDoc

Probably not: The goals for BP are looser for older, frailer seniors. In addition, the actual BP may be grossly overestimated due to stiff blood vessels which make it difficult to get a true reading. If there is no evidence of 'end organ damage'(heart, kidney disease)- we maybe more relaxed with this bp. But overall, this is too high for good health.

Look into reasons: I would suggest you look into reasons you have elevated BP - stress, anxiety, white coat syndrome? Medications? family history? smoking? alcohol etc. These can all affect BP and you should go over these issues with your MD.

Also, if the thought of having hypertension (high blood pressure) bothers you, start cutting down the salt, optimizing your weight and the quality of your diet (low fat, high fiber, variety of fruits and veggies ), back off alcohol, and don't smoke. Add light exercise until you have the blood pressure rechecked and your MD okays it. Check and see if any over the counter meds may affect BP.
