Doubt epididymitis: "Slight testicular pain" usually not due to infection. True infection (epididymitis) usually has severe pain and testicular enlargement. Your doctor likely gave Vibramycin (doxycycline) as a trial for possible epididymitis due to chlamydia, but non-improvement is additional evidence against it. But the antibiotic makes current chlamydia test meaningless. Maybe chronic pelvic pain syndrome?

Possible infection: Commonest cause of testicular pain is infection. Urine is most of the times normal and your doctor has already tested you for clamydia, which is a good thing. He will still need to prescribe you an antibiotic to treat the infection. This will also help with your pain.
