Different toe Box.: So let me see. The outside of the big toes and the outside of the little toes, both hurt and are a little red. The first place I would be looking for a cause is your shoes. The areas in question are the areas that that receive the most pressure from shoes that are not fitting your foot right. Red dry looking and aching are the warning signs of ingrown toenails, or at least inflamed nail borders.
