In the course of...: sexual activity many illnesses, diseases and injury can occur. This scenario, in the absence of other symptoms mentioned, seems most likely to me from strained skeletal muscles. I would agree infection is unlikely plus pain from the kidneys is typically in the flank/back lateral to midline.
