Two separate: questions, although if you don't treat forgetfulness, you might not remember to treat the wart! Seriously, go to a dermatologist for the former. The latter is more complicated and can be caused by LOTS of conditions. Start with a general physical and fine-tune from there. Don't want to speculate further because there are too many possibilities. (Debating in a vacuum.)

These r 2 separate ?: I will leave the wart to the dermatologist. Memory problems usually don't develop in people your age so you want to be evaluated. Your doctor will test your memory and determine if it suggests a serious problem. It may be necessary for a neurological consultation and brain imagining . Sometimes it turns out that the problem can be related to anxiety and poor concentration. Talk to your doctor.
