Not sure what you me: Palpitations can be sustained or an extra beat with slight irregularity. It is not clear to me what you exactly have. It sounds as though you need some monitoring during the time that you have this palpitation. It is likely to be an extra beat. It is not clear to me if you've had monitoring performed at the time of the palpitations but that is what you need to have.
