Peroneus quartus : Peroneus quartus is a fairly uncommon presence of an extra muscle belly in the lateral ankle. It can be the source of lateral ankle pain. This extra muscle has been present in your ankle all your life, so running a marathon will not likely cause any permanent damage to your ankle, but may lead to worsening of lateral ankle pain if the area is already irritated
