Emergency: Please either see your podiatrist, orthopedic surgeon, or emergency room doctor right away. Pain an discoloration in the toe of a patient with diabetes could be from poor circulation and possible infection.

Infection: Pain + discoloration in the toe for a diabetic is considered emergent and needs to be treated asap. The problem can be an infected ingrown nail, trauma, gout, or circulation. If it is a nail or soft tissue infection, oral antibiotics need to be started asap. If the infection is in the bone, then IV antibiotics are necessary. Mri can confirm. Stabilize trauma. Treat the gout. Act fast=save the toe.

Many possibilities: Your pain could be as a result of many different things. Causes of pain may include: trauma (fracture, sprain, arthritis), infection (ingrown nail), metabolic conditions (gout), musculoskeletal or biomechanical abnormality and neurologic conditions (neuropathy). A thorough history and physical exam and possibly some laboratory and radiology exams are needed to determine the exact cause.
