Are there any circumstances that would make you recommend treatment for dcis in a 90 y.o. woman?

Yes: If the DCIS was coming from a palpable mass and pathology showed high grade features, then I would consider lumpectomy if the patient had good health otherwise. This would be to avoid the development of a large mass that might erode the skin and cause a whole other set of problems. I would not recommend the other parts of treatment, like sentinel lymph node biopsy or chemo
