Time is now: Having the testis brought down soon solves several issues.In most they descend by 34 wks in pregnancy, if still up at 4,they won't come down without surgical help.Proper location helps preserve fertility and eases self exam for cancer (a big issue). The procedure itself is delicate and duration as long as needed (hours). Minimal risks in experienced hands.(anesthesia,infection,testis)

Surgery mandatory: Surgical approach differs between various pediatric urologists & pediatric surgeons. Also depends on scrotal size & would stage if scrotum is tiny & flat. Surgery can be done laparoscopically, trans-abdominally or by inguinal approach. Latter, which I don't do because of possible difficulty in getting sufficient length of spermatic cord. Surgery 4 1 takes 1 1/4 - 1 1/2hrs, 2 - 2 1/2 hrs for both.
