An approach....: First make sure you don't have a severe life- threatening illness preventing you from gaining weight. That will require seeing a clinician. Once done consider other variables: dietary protein, digestion, absorption. I start with a plant based, anti-inflammatory, low glycemic, hormonally correct protein powder (1-2x/day) that is also chock full of vitamins, trace minerals, and enzymes.
