The thumping sound: in the ear suggests the cause is turbulent blood flow. This may come from an obstruction of an artery near the ear, or an AVM/arteriovenous malformation where artery and vein connections meet. Because you are also dizzy there is more concern. See your doctor as soon as possible for an evaluation. While unlikely, an impending stroke assessment should be done. Good wishes:)
