Topical hemorrhoid: treatments, often containing witch hazel or hydrocortisone, are often effective. However, sometimes more is needed. Sometimes a blood clot forms within an external hemorrhoid; your doctor can remove it with an incision. Additional treatments include rubber band ligation, injection/sclerotherapy, or laser or infrared coagulation. Ultimately surgical removal/excision can be done. Good wishes

See below: The pain is due to small blood clot (thrombosis) in the hemorrhoid. The pain may last for 4-7 days and gradually the clot disappears. Immediate relief for the pain can be had by the physician making a small incision(using local anesthetic) in the hemorrhoid releasing the clot. Topical hydrocortisone/lidocaine cream can help the pain.OTC products with 5% Lidocaine and hydrocortisone are available
