First make sure: There is none. Hair or string wrapped around a toe does happen and can lead to auto amputation of the digit. This is known as ainhum. If nothing is there, check under the digit to make sure there is no small crack in the skin. Make sure shoes are proper fit. If all above check out I'd consider neuropathy.
