Emergency room: Testicular pain requires emergency room evaluation to make sure it is not torsion. Other common conditions like epididymitis or UTI.

Acute pain: If you have acute testicular pain and is severe then seek care now. Torsion is a concern and time is of essence. Infection is the second issue and again need to seek care. So yes ER it is. Best of luck.
