Abscess?: It looks like it may be developing into an abscess, though it's impossible to say for sure based on a photo. It's certainly something that should be seen in person. Urgent care can easily handle this. Likely will need antibiotics, possible drainage. This is something that can recur, spread or worsen, so hygiene is VERY important.
