Many things: Get it checked out. Is it enlarging? Did he have it from birth? Could be a cyst, a sinus etc. Get it checked out

Could be lymph node: This is the location of an enarged post auricular lymph node. It coudl be somethign else but you can clarify thsi with your pediatrician.
