Can a gastric/duodenal/esophageal ulcer cause chronic middle back pain and fatigue/tiredness?

Yes, sure can!: An ulcer in any of these places can cause back pain and fatigue. Other causes are possible, too. So treat the ulcer. If the back pain and fatigue does not go away, then get checked for other possible reasons.

Intestinal ulcers: esophageal ulcers tend to cause chest and upper abdominal pain. Gastric ulcers depending on location can cause upper abdominal and per umbilical pain. Duodenal ulcers can cause mid abdominal pain that radiates to the back. As the ulcer heals the pain should subside.
