See a : Podiatrist, you likely need a partial nail avulsion under local anesthesia with or without chemo cautery if the granulation tissue.

Painful granulation: This likely needs to be removed. There has likely been some type of toe irritation, trauma or possible ingrown toenail. Either way this needs to be checked out, possible biopsied, but should be easily treatable if it is just granulation tissue.
