It depends on many: factors. Your pain tolerance, the size and location of the skin tag. something along the shaft should not be as tender as something on the glans (end). If this is superficial, a small injection of lidocaine will take all pain away, just like it does at the dentist. Small ones are often just snipped off with a sharp scissors.
