Need a picture: to judge, at any rate under- is better than over-correction, if in doubt ask a urologist to have a look, so long he is passing urine, no urgency, you can observe

Incomplete: This is an incomplete circumcision. The entire glans (head) of the penis should be out after the circumcision is done. It is not a medical problem, as circumcision is optional. I would have his penis reevaluated for possible revision or to teach you to avoid adhesions.
