Consult: with a urologist or dermatologist. Any skin lesion that persists for this long needs specialists' attention. It is most likely some sort of infection, but skin cancer can occur on penis, however rarely.

Showed up from: nowhere !!. I would think also psoriasis among other possible causes certainly yeast infection would not be among them since lotrimin did not work. But my feeling is that if it was lotrisone (clotrimazole and betamethasone) instead of lotrimin that would help. You need corticosteroid in here. See a dermatologist .
