No other choice: This is an extremely premature child and they probably have to do the procedure if he failed medical therapy to close the PDA. The risk to his/her life of not doing anything is greater than the risk of the surgery

PDA: For an extremely premature baby all surgeries carry significantly elevated risks. The good news about all of this is: IF medical therapy fails (indomethacin) the PDA surgery is not "open heart surgery", the CT surgeon approaches the PDA through the side and places a clip on the ductus thus closing it off. Ignoring the ductus is not an option.
