Blockage: Blockage at multiple small bile duct levels by cancer can cause bilirubin to go up. Could be liver failing if you had cirrhosis/ or other liver conditions before.

See prior answer: The small mets are not permitting the smaller bile ducts to drain. This will progress as the tumor mets enlarge
