Need exam : Too complex to sort out on a short answer online forum. See a gynecologist for a proper evaluation. Whole there, ask if a referral to a colorectal surgeon for the hemorrhoids is appropriate. They usually only operate as a last resort, and are very knowledgeable about non surgical therapy.
