Who takes out a nephrostomy tube a radiologist or my urologist. also do they give me medicine like they did to put it in or just pull it out?

Either one: Either one can do it. Usually the urologist is the one who decides when is the proper time for removal, so he or she usually does it. It usually just slides out, no medicine is needed.
