Neck pain: Yes. With a history of fusion of the cervical spine you need a mri. You are having problems consistent with a entrapped nerve. Get the mri.

More than that: 54 yo has severe shoulder pain with distal parasthesias & weak hand; with symptom onset after an acute shoulder injury. This requires urgent attention with concern for brachial neural plexus injury or neurally compressive collections of blood such as vascular clots or hematomas. Surgical intervention might be necessary to avoid permanency of these symptoms. Yes, an MRI is necessary in this case.
